Service Requests
Registrants can request the following services from the College:
- Certificate of Standing confirms your current registration status with the College of Opticians of Ontario and contains detailed information about your registration history and any applicable complaint investigations and disciplinary history. Upon receiving your request, the College will send the Certificate of Standing to the requested organization. The certificate is only considered valid if it has been received directly from the College. The cost for a Certificate of Standing is $25 + HST.
- Replacing Documents due to loss/damage, or name change.
- Certificate of Registration: $50.00 + HST
- Certificate of Registration Decal: $25.00 + HST
- Photo Identification Badge: $50.00 + HST
- Photo Identification Badge Decal: $25.00 + HST
How to Make a Service Request
To request a Certificate of Standing, you will need to determine which type you need. The College issues two types:
- Issued to a regulatory body and intended for those seeking registration in another jurisdiction. To request this, please click here to download and fill out this form.
- Issued to a non-regulatory body to confirm your registration with the College for other purposes such as applying for a job, volunteering, submitting or verifying insurance claims, or enrolling in a training program. To request this, please click here to download and fill out this form.
- Once you have completed the Certificate of Standing form and emailed it to [email protected], you can make payment via the registrant portal. To pay, please:
- Log in to the registrant portal.
- Along the top, please click on "Documents & Fees".
- Under the "Fees" section, please click "Request Document or Service".
- Scroll to the desired request and click "Add to Cart".
- Scroll to the bottom of the page and click "Proceed to Checkout".
- It will then take you to a payment page to make the payment.
To request either a replacement Certificate of Registration, a replacement photo identification badge, or replacement decals, please:
- Complete the Document Request form and email it to [email protected] for processing.
- Pay the service request fee. To pay, please:
- Log in to the registrant portal.
- Along the top, please click on "Documents & Fees".
- Under the "Fees" section, please click "Request Document or Service".
- Scroll to the desired request and click "Add to Cart".
- Scroll to the bottom of the page and click "Proceed to Checkout".
- It will then take you to a payment page to make the payment.
Processing Time
Please allow up to 10 business days for the College to process your request. Mailing time with Canada Post is not included in this estimate. Fees are non-refundable.
